The SHIFT-Care Challenge

The SHIFT-Care Challenge invested funds for competitively-selected Massachusetts providers to promote sustainable, transformative care models that seek to reduce avoidable acute care utilization.

Overview

On January 9, 2018, the HPC made up to $10,000,000 available for the SHIFT-Care Challenge. The SHIFT-Care Challenge (SHIFT-Care) represented a significant investment on behalf of the Commonwealth in sustainable, transformative care models that sought to reduce avoidable acute care utilization. Through SHIFT-Care, grantees received funds to invest in promising innovations that addressed health-related social needs and increased access to timely behavioral health services for residents of Massachusetts with the goal of decreasing the use of costly and avoidable hospital care.

Eligible entities applied for up to $750,000 to prepare and implement an initiative focused on one of two areas during a 21-month period of performance:

  • Addressing health-related social needs
  • Increasing access to behavioral health care

At the July 18, 2018 Board Meeting, HPC staff presented on the SHIFT-Care award recommendations
