Promoting innovative care and payment models.

To complement its research agenda and market oversight roles, the HPC oversees a range of initiatives aimed at encouraging effective and efficient care through adoption of innovative care delivery and payment models. Through two primary vehicles – strategic investments and certification programs – the HPC works in partnership with provider organizations to promote a more transparent, accountable, and equitable health care system.

The HPC partners closely with state agencies and other stakeholders to advance these shared goals, providing funds, expertise, and/or staff resources on projects ranging from aligned quality measurement to promoting maternal health in the Commonwealth.

Three Branches of Care Delivery Transformation

Certification Programs

The HPC’s certification programs set standards for Accountable Care Organizations (ACO) and Patient-Centered Medical Homes (PCMH) in order to promote the adoption of new delivery system models in the Commonwealth.

Investment Programs

Since 2014, the HPC has invested over $100 million in health care organizations across the Commonwealth to enhance the delivery of effective, efficient care, promote innovative models, and work with communities to address the social determinants of health.

Partnerships

The HPC regularly seeks opportunities to partner with state agencies and other stakeholders to advance shared goals for health care transformation and innovation. By providing funds, expertise, and/or staff resources, the HPC can contribute broadly to efforts to promote a more transparent, accountable, and equitable health care system.
