Health Care Innovation Spotlight: Baystate Medical Center Springfield Healthy Homes Asthma Program

The HPC’s SHIFT-Care SHIFT-Care Investment Program provided funding for Baystate Medical Center and PHIWM to implement an evidence-based home visit and remediation service that aimed to reduce acute care utilization and out-of-pocket health care costs, and improve the quality of life for people living with asthma.
