Supporting Individuals with Unmet Health-Related Social Needs and Behavioral Health Needs: The SHIFT-Care Challenge Investment Program

This report provides an overview of the SHIFT-Care Challenge Investment Program, which supported innovations to address health-related social needs and increase access to timely behavioral health services, with the goal of decreasing the use of costly and avoidable hospital care in Massachusetts.
