Health care delivery transformation achievements of Massachusetts community hospitals to be featured
BOXBOROUGH — The Massachusetts Health Policy Commission will host a day-long conference (Agenda) featuring successes and lessons learned among the Community Hospital Acceleration, Revitalization, and Transformation (CHART) Phase 2 grant recipients at the Boxboro Regency Hotel and Conference Center in Boxborough, MA. The event will highlight the individual and collective work of the two-year, $60 million investment the HPC made in 25 hospital programs to improve care for high risk and vulnerable populations. The event also includes panel discussions and breakout sessions where attendees will hear results directly from care team members and hospital leadership implementing these programs.
Attendees will share and learn about strategies and tactics for:
- Reducing avoidable readmissions and emergency department (ED) visits for patients with complex medical, behavioral, and/or social needs;
- Improving care for behavioral health patients in the ED; and
- Integrating, sustaining, and expanding upon the effective capabilities and operations developed for system-wide impact.
