Supporting Partnerships Between Health Care Providers and Community Organizations To Address Social Determinants of Health

The Moving Massachusetts Upstream (MassUP) Investment Program supported partnerships between hospitals, community-based organizations, and residents aimed at addressing social determinants of health (SDOH). Over three years, Heywood Hospital, Massachusetts General Hospital, Cooley Dickinson Health Care, and Mercy Medical Center partnered with community-based organizations to work toward improvements in two SDOH affecting their local communities: food security and economic stability.

This impact brief shares key findings and lessons from each MassUP partnership. The experiences of MassUP partnerships offer insights into building healthier communities by addressing the root causes of health through cross-sector collaboration, meaningful resident engagement, and investments in upstream strategies.
